Many Carers reach a turning point, a crossroads, at which they feel they need help with caring, a sympathetic ear or sometime to themselves. This is what Crossroads does.
How to book a break
You can contact us at anytime to book a break. You do not have to be referred by a Social Worker or GP. You can contact us at anytime by calling 01782 268391.
What happens next?
You will be introduced to one of our Care Coordinators, who will arrange a date and time that suits you, to visit yourself and the person you are caring for in your own home or place of residence. The Care Coordinator will work in paternship with you both, to make a care plan that details your needs, wishes and preferences. We can arrange advocacy or an interpreter if this will help you to communicate your circumstances. Once you are both happy with your care plan, your Care Cordinator will ask when you would like care and support and your preferences on a team of Carer Support Workers. We provide care and support 365 days a year, over a 24/7 period. You can choose times that suit you. This could be mornings, evenings, day times, weekends and/ or overnight care and support.
Allocation of a Carer Support Team
Our Carer Support Workers have been carefully selected and trained to a high standard to provide quality care and dependable support All of our Carer Support Workers are trained highly, meeting and exceeding qualifications detailed in The Care Act and have been assessed as competent through their Care Certificates.
We know that a regular and consistent team, is essential to the families we support. This is why we will introduce a small team to you and we will match this dependent upon your preferences, what you like to so, such as your hobbies and interests.
Our dedicated team are trained to deliver all aspects of personal care, including continence, medication and specialised complex health care tasks.
Your care package will be overseen by your Care Coordinator who will visit you regularly to gain your feedback about the service you receive and to plan to any change in needs and circumstances.
